How to Take Epkinly
Epkinly is administered as an injection under the skin by a healthcare professional, following a specific dosing schedule.
Dosage Instructions
The first full dose of Epkinly (48 mg) is given on Cycle 1 Day 15. Treatment is typically given in cycles: Weekly for Cycles 1 to 3, every other week for Cycles 4 to 9, and every four weeks for Cycles 10 and beyond.
What If You Miss a Dose?
If you miss your appointment for treatment, contact your healthcare provider immediately to reschedule.
Important: Do not double dose if you miss an appointment.
Side Effects
Like all medications, this medicine can cause side effects. Most are mild and temporary, but some require medical attention.
Common Side Effects
These side effects occur relatively frequently and usually don’t require emergency care:
- diarrhea
- fever due to infection (febrile neutropenia)
- headache
- injection site reactions
- itching
- nausea

- tender swollen lymph nodes

- cough or difficulty breathing
- vomiting
Seek immediate medical attention if you experience:
- Cytokine Release Syndrome (CRS): fever, dizziness, chills, fast heartbeat, difficulty breathing
- Haemophagocytic lymphohistiocytosis (HLH): fever, low blood cell counts, easy bruising, liver problems
- Immune Effector Cell-Associated Neurotoxicity Syndrome (ICANS): difficulty speaking, drowsiness, confusion, muscle weakness, seizures
- Serious infections: fever of 38°C or above, chills, cough, chest pain, shortness of breath, painful rash, sore throat, pain with urination, feeling weak
- Tumor Lysis Syndrome (TLS): fever, chills, vomiting, confusion, shortness of breath, seizures, irregular heartbeat, dark or cloudy urine, unusual tiredness, muscle or joint pain

Do not use Epkinly if you have had an allergic reaction to epcoritamab or any of its ingredients.

Discuss any history of nervous system issues, current infections, or if you are planning to get vaccinated with your doctor.
Pregnancy and Breastfeeding
Pregnancy must be ruled out before treatment. Women should use effective contraception during treatment and for at least 4 months after the last dose. Breastfeeding is not recommended during treatment and for at least 4 months after.
